What is to be Serviced:
Pool Doctor agrees to perform all the services necessary to maintain the facilities, as specified below in a clean, sanitary and presentable condition by the performance of work detailed below:
*All necessary chemicals needed to balance pool water are included; Chlorine, Acid, Bi--carb, and calcium.
- Test pool water
- Chlorinate pool
- Adjust pool PH
- Balance total alkalinity
- Vacuum and brush pool
- Clean tile
- Skim pool surface
- Clean filter (as required)
- Keep equipment room clean
- Fill out health department reports per H.R.S. requirements
- Inform customer of all necessary repairs
Insurance
Pool Doctor will submit evidence of worker's compensation and liability insurance upon request.
Term
This agreement shall be for for a period of 1 year(s). This agreement is cancelable by either party by giving written notice within thirty days.
Payment
Payment will be made on a monthly basis. All repairs will be billed and paid upon invoice from pool doctor.
